Revised Activity Prescription Form available Nov. 3, 2014.
L&I has revised the APF, which communicates changes in an injured worker’s medical or work status. Claim managers use the APF to make timely decisions about services and benefits.
Changes effective Jan. 1, 2015
Providers can “self-generate” the form when a worker’s status changes. Note that claim managers, vocational counselors, and employers can still request an APF.
Billing limits: each provider may submit up to 6 APFs per worker within the first 60 days of the initial visit date and then up to 4 times per each 60 days thereafter.
L&I is retiring code 1069M. For dates of service on or after January 1, 2015, COHE program and ortho/neuro surgeon quality project providers must bill for the APF using code 1073M.
